Don't sleep with a heating pad for a backache - not only because it can burn you, but too much concentrated heat can exacerbate the problem.
If you want to treat the backache, it works better if it can be moist heat. Like a damp towel between you and the hot pack. About 20 minutes. Even better, alternate between ice (done the same way with a warm moist towel - helps reduce the shock of the cold and lets it penetrate better) and the heat. 20 min hot, 10 ice, 30 minutes of nothing. Repeat if you want. And stretch. That can help a lot.
The back and forth of cold and heat does a better job of getting everything circulating and healing. Course not until your blister heals! And no more sleeping on things that can burn you! So sorry you are having to deal with that!
